Silicon Hills (http://www.siliconhills.com/) speciailizes in exactly that type of work. I have personally used them to layout PCBs as complex as a ten layer board with several hundred impedance matched, shielded pairs. Although they are also involved with fabricating & populating PCBs, design seems to be the focus of their business.
