3D Nurbs surface machining

Does anyone have experience with 3D CNC machining of complex surfaces
such as inner cavities and cores? 3D cutting coupled with high speed
machining is a prominent form of RP&T, so I wanted to post the question
here. What speeds and feeds, ball mill diameter, min stepover or cusp
height, do you pre-relieve inner surface intersections and how, etc...
I have been using NC Polaris inside AutoCAD MDT and getting some
impressive but inconsistent results. I heard some Mold & Die shops can
achieve almost mirror-like surfaces which require practically no
bench-work.
